FAA Taking Steps to Correct DFW Operational Error Misclassifications
The FAA says it's taking steps to strengthen the reporting system designed to classify airspace errors,
after a report by the U.S. Department of Transportation's Inspector General (IG)
revealed there have been intentional misclassifications of operational errors at
Dallas-Fort Worth's TRACON (terminal approach control). The action will include new reporting automation.
The FAA responded to the report by removing both the TRACON's facility manager and assistant manager from
their positions pending a final determination on possible further personnel
actions. It also indicated that additional personnel actions might be taken.
In addition, the FAA's
Air Traffic Safety Oversight Service will make unannounced on-site audits at
the TRACON, and require monthly reports to the FAA's acting administrator. And
the agency will accelerate deployment of its TARP (traffic analysis review
program) software that automatically detects losses of aircraft separation at
terminal facilities, so it can be implemented by the end of the government's
